Thanks to pixelco I found 3 excellent tools to create online formularies

JotForm This tool comes in 2 versions, one free with a limited 100 formularies, 3 for insurances, 3 for payments and 10MB per month. The paid version has tech support, forum, 1000MB and unlimited formularies. JotForm is the best tool I’ve ever found.

FormSite, with this you can create formularies and compilate information and even make result analisis. Many options, many formularies, multi-language support, reports, image upload, etc…

formspring this is an idea tool for: marketing campaigns, surveys, shopping formularies and register.
